Esketamine (Spravato): Depression Treatment and Monitoring

Esketamine nasal spray is a supervised treatment for defined depression indications. It is distinct from a take-home nasal product or an unsupervised ketamine treatment. Selection, clinic observation, transport and continuing care are central to both safety and the usefulness of treatment.

Esketamine nasal spray is a supervised treatment for defined depression indications. It is distinct from a take-home nasal product or an unsupervised ketamine treatment. Selection, clinic observation, transport and continuing care are central to both safety and the usefulness of treatment.

Indications and distinction from ketamine

The US Spravato indication includes treatment-resistant depression in adults, used alone or with an oral antidepressant under the current label. A separate indication involving depressive symptoms in adults with acute suicidal ideation or behavior has its own treatment requirements. Local indications and authorization may differ.

Esketamine is related to ketamine, but products, routes, evidence and regulatory requirements are not interchangeable. The assessment should review previous depression treatment, current symptoms, co-occurring conditions and alternatives. A rapid symptom response does not establish lasting recovery or replace a broader treatment plan.

Assessment and supervised administration

The clinician considers cardiovascular and cerebrovascular risks, blood pressure, breathing, substance-use history and other medicines. Certain vascular conditions and previous intracerebral hemorrhage are important contraindication considerations. Pregnancy and breastfeeding require specific assessment under the product guidance.

Spravato is administered in an authorized clinical setting with observation for at least two hours under the US program. Sedation, dissociation and breathing effects can occur. The service should explain monitoring, transport and the requirements before a visit, rather than treating administration as a routine prescription pickup.

Side effects and urgent concerns

Dissociation, dizziness, nausea, sedation, changes in perception and increased blood pressure may occur. The clinical team monitors these effects and determines when discharge is appropriate. A previous visit without problems does not remove the need for observation at later treatments.

Severe breathing problems, chest symptoms, a major neurological change or another serious deterioration needs urgent assessment. Current suicidality also requires a safety plan. Evidence of improvement in depressive symptoms does not prove that esketamine prevents suicide or that hospitalization or other urgent care is no longer needed.

Interactions, driving and daily arrangements

Sedatives, opioids and alcohol can increase impairment; stimulants and other treatments may contribute to blood-pressure concerns. A complete medication and substance-use review is essential. The team should also explain product-specific food and drink instructions before treatment.

Follow the driving restriction in the current label, which includes waiting until the following day after a restful sleep in the US guidance. Arrange transport and practical support in advance. Feeling more alert immediately after observation is not permission to drive or undertake hazardous tasks contrary to the instructions.

Response review and longer-term care

Review depressive symptoms, functioning, tolerability and the ongoing treatment plan. A service should explain how benefit is assessed, how further sessions are decided and how other depression care continues. Cost, travel and reliable attendance are legitimate parts of the shared decision.

Do not obtain or use an unsupervised product as a substitute for the authorized treatment. If appointments are interrupted or benefit is limited, discuss next steps with the specialist. Psychological care, relapse prevention and treatment of co-occurring conditions remain relevant even when symptoms improve quickly.

Treatment-resistant depression and the previous treatment history

Before esketamine is considered, the team should review the diagnosis and what earlier treatments actually showed. A medicine stopped because of severe adverse effects is different from an adequate trial with insufficient benefit. Supply problems, inconsistent use or another untreated condition can also affect the interpretation. Bring the history as accurately as possible rather than describe every earlier prescription simply as a failure.

In the US, Spravato's treatment-resistant depression indication allows monotherapy or use with an oral antidepressant under current labeling; other indications have different requirements. Local approval and service criteria may differ. Ask which indication applies and what ongoing care is planned. The antidepressant overview and depression treatment guide provide broader context. A rapid-acting treatment still needs a defined goal and follow-up, and an early symptom change should not be presented as a guarantee of lasting recovery or prevention of suicide.

What a supervised esketamine visit involves

Esketamine nasal spray is administered in a supervised clinical setting with assessment and monitoring, not supplied as an unsupervised home treatment. The service should explain the visit, required observation and criteria for leaving. US treatment involves monitoring for at least two hours, while the actual visit may require more time depending on the person's condition. Ask about transport, work and caregiving arrangements before starting.

Dissociation, sedation, blood-pressure changes and other effects can occur. Tell the team about your medical history and all substances or medicines that could add to impairment. Driving restrictions extend beyond simply feeling awake after the appointment; follow the product and service instructions, including the need to wait until the next day after restful sleep. A trusted person may help with transport if appropriate. Planning these practical commitments is part of deciding whether treatment can be carried out safely and consistently, not an administrative detail to solve after the first dose.

Esketamine versus ketamine treatments

Esketamine is one form of the ketamine molecule, but an approved nasal-spray product is not interchangeable with every treatment described as ketamine therapy. Route, formulation, evidence, regulatory status and monitoring differ. A clinic's use of an off-label ketamine approach should be explained separately rather than treated as the same approval as Spravato.

The ketamine resource provides background on the substance, while the prescribing decision requires the exact proposed treatment and local clinical framework. Ask about the indication, evidence, supervision and continuing plan. Do not use recreational ketamine or an unverified home product as a substitute for supervised treatment. A prior response to one route does not automatically predict another regimen's effectiveness or safety. The team should compare suitable options using the depression history, physical health, risks and practical access, with clear information about uncertainties instead of grouping all ketamine-related interventions into one promise.

Rapid improvement, crisis care and longer-term follow-up

A rapid reduction in depressive symptoms can be valuable, but it does not mean that risk assessment or continuing care can stop. Esketamine has not been established as a treatment that prevents suicide or removes the need for hospitalization when indicated. If someone has immediate suicidal intent, cannot stay safe or deteriorates significantly, emergency and crisis care remain necessary regardless of a scheduled treatment appointment.

Discuss how response will be assessed across the treatment period and what happens if benefit is partial, short-lived or absent. Psychotherapy, other medicines and support for daily functioning may remain important. A plan should identify the responsible clinician after a course or change in treatment frequency. Do not interpret a difficult week as permission to obtain extra treatment elsewhere. Coordinated follow-up should assess symptoms, adverse effects, functioning and personal goals, with a clear route for urgent help. The value of a specialist medicine lies in its role within this wider plan rather than speed of action alone.

Preparing for a specialist treatment decision

Specialist medication decisions often require more information than a diagnosis alone. Bring a treatment history describing which medicines were tried, the reason for each, how they affected symptoms, and why they were stopped. An earlier trial may have been limited by adverse effects, access problems or an inadequate opportunity to assess benefit. These distinctions help the team decide what the existing history actually shows.

Ask how the proposed medicine differs from the treatments you have already used and what uncertainty remains. Discuss the expected setting, monitoring, practical commitments and alternatives. A new mechanism or recent approval does not guarantee a better personal outcome. Equally, a previous unsuccessful treatment does not mean that no further option can help. The decision should connect the evidence with your symptoms, medical history and priorities.

Make sure you know who is responsible for follow-up, prescription supply and advice between visits. Some specialist treatments require coordination with primary care, a pharmacy or a supervised clinic. If travel, work or caregiving makes the proposed schedule difficult, raise that before treatment begins. A plan that looks appropriate on paper still needs to be feasible in daily life. Ask for the current product information and use it alongside the clinician's explanation, especially when availability or approved indications differ between countries.

Questions

Frequently Asked Questions

Can prescribed esketamine be used at home without observation?

Spravato treatment requires supervised administration and monitoring in an appropriate clinical setting. It is not an unsupervised home nasal-spray regimen. Ask the service about observation, transport and driving restrictions before beginning. Follow the exact local product requirements and do not substitute recreational ketamine or an unverified product for the prescribed treatment.

Does feeling better quickly mean crisis care is no longer needed?

No. Symptom improvement does not establish prevention of suicide or remove the need for ongoing risk assessment and hospitalization when appropriate. Immediate danger, suicidal intent or inability to stay safe requires urgent help. The treatment team should explain both the medication's goal and the continuing safety plan, including contacts between supervised visits.

Can Spravato be used without an oral antidepressant?

The current US treatment-resistant-depression indication permits use alone or with an oral antidepressant. Other indications and countries can have different requirements. The specialist must apply the current local label to the person's diagnosis.

Is it the same as an IV ketamine infusion?

No. Related pharmacology does not make the product, route, evidence or service requirements identical. Ask the specialist to distinguish the proposed treatment, authorization, alternatives and follow-up.

Does improvement prove that suicide risk has ended?

No. Symptom improvement does not establish suicide prevention or remove the need for ongoing assessment. A person with current suicidal thoughts needs an appropriate safety and care plan, including urgent support when indicated.

Can I drive home after the observation period?

No. Arrange transport and follow the current product's driving restriction. Under US guidance, driving is deferred until the following day after a restful sleep. Clinic discharge and permission to drive are separate decisions.
