Side Effects Of Xanax

Xanax (alprazolam) can cause drowsiness, impaired coordination and memory problems. Continued use can cause physical dependence, while mixing it with opioids, alcohol or other sedatives can lead to dangerous breathing problems or overdose. Do not stop suddenly after regular use.

Quick Summary

  • Xanax (alprazolam) is a benzodiazepine that enhances GABA-A receptor activity and is prescribed for specific anxiety or panic indications that vary by product and country.
  • Common effects include drowsiness, dizziness, impaired coordination and memory problems; opioids, alcohol and other sedatives can greatly increase overdose and breathing risks.
  • Continued use can cause physical dependence without necessarily meaning addiction. Do not stop suddenly; any dose reduction should follow an individualized clinical plan.

Xanax (alprazolam) can cause drowsiness, impaired coordination and memory problems. Continued use can produce physical dependence, while misuse can lead to addiction, overdose or death. Physical dependence is not the same as addiction, and alprazolam should not be stopped suddenly after regular use.

Xanax is a brand of alprazolam, a prescription benzodiazepine. It enhances the effect of the inhibitory neurotransmitter gamma-aminobutyric acid (GABA) at GABA-A receptors; it does not simply increase the amount of GABA in the brain. This central nervous system effect can reduce anxiety but can also cause sedation, slowed reactions and impaired coordination.

How Does Xanax Work In The Body

Alprazolam binds to a regulatory site on GABA-A receptors and makes the receptor more responsive to GABA. Effects and duration vary with the formulation, dose, other medicines, age, liver function and individual response. Do not use how quickly a dose feels effective as a reason to take more.

Why Are People Prescribed Xanax

Approved indications differ by product and country. The current US alprazolam tablet label includes acute treatment of generalized anxiety disorder in adults and treatment of panic disorder, with or without agoraphobia. Alprazolam is not a general treatment for everyday stress, and it is not routinely indicated for insomnia or social anxiety disorder. Treatment should be regularly reviewed and taken only as prescribed. See the current DailyMed prescribing information.

Common Side Effects In Adults

Commonly reported effects include drowsiness, dizziness or light-headedness, impaired coordination, fatigue, memory or concentration problems, and changes in speech or balance. They can affect driving and increase the risk of falls. Do not drive or operate machinery until you know how alprazolam affects you.

When Side Effects Need Urgent Care

Seek prompt medical advice for hallucinations, marked confusion, unusual agitation, worsening depression or suicidal thoughts. Call emergency services for slowed or difficult breathing, extreme sleepiness, collapse or inability to wake the person, especially if alprazolam may have been combined with an opioid, alcohol or another sedative.

Do Side Effects Go Away Over Time

Some effects such as sleepiness may lessen, but this is not guaranteed. Tolerance can develop to some effects and does not make dose escalation safe. New, persistent or worsening symptoms should be reviewed by the prescriber; never increase, reduce or stop the dose without an individualized plan.

How Xanax May Affect Women

The core adverse effects of alprazolam apply to people of all sexes. Available prescribing information does not support a simple rule that women experience more severe side effects because of menstrual or menopausal hormone changes. Individual factors (including dose, other medicines, liver function, age and pregnancy) are more useful when assessing risk.

Pregnancy And Breastfeeding

Benzodiazepines cross the placenta. Use later in pregnancy may cause neonatal sedation, breathing problems, feeding difficulty or withdrawal. Alprazolam also passes into breast milk and may affect a nursing infant. Anyone who is pregnant, planning pregnancy or breastfeeding should discuss the risks and alternatives with the prescriber. Do not stop alprazolam suddenly, because withdrawal can be dangerous.

Mood And Cognitive Effects

Depression, emotional blunting, memory problems and difficulty concentrating can occur in any patient. Rebound anxiety or withdrawal symptoms may appear as a dose wears off or after a rapid reduction. These symptoms need clinical review and do not by themselves establish addiction.

Side Effects In Men

Men generally face the same sedation, coordination, memory, dependence and interaction risks as other adults. Changes in libido or sexual function are reported with alprazolam, but evidence does not support ranking these effects as reliably worse in men.

Physical And Psychological Effects

Fatigue, muscle weakness, mood changes, agitation or emotional blunting may occur. Their presence and severity depend on the individual and other health or medication factors, not on assumptions about how men cope with stress.

Long-Term Use

Continued use can lead to physical dependence and may be associated with ongoing cognitive or mood symptoms. Physical dependence is an expected physiological adaptation in some patients and is distinct from addiction. Routine therapeutic use is not established as a cause of progressive liver damage, although liver disease can slow alprazolam clearance and may require different dosing or monitoring. Alcohol increases sedation and overdose risk and should be avoided.

Why Older Adults Are At Higher Risk

Older adults may clear alprazolam more slowly and can be more sensitive to sedation, confusion, impaired coordination and memory effects. These effects increase the risk of falls and injury. Other medicines and medical conditions can further increase risk, so the medication list and continuing need for treatment should be reviewed regularly.

Cognition And Function

Alprazolam can cause acute memory and attention problems. Observational studies of benzodiazepines and dementia cannot prove that the medicine causes dementia, because the conditions being treated and other factors may influence the association. New confusion, functional decline or excessive sedation warrants prompt review.

Falls And Driving

Drowsiness, dizziness and poor coordination can make falls, fractures and driving injuries more likely. Prescribers may use a lower starting dose in older adults and should reassess benefit, adverse effects and safer alternatives. Patients should not drive or operate machinery while impaired.

Physical Dependence, Withdrawal And Overdose Risk

Continued alprazolam use can cause clinically significant physical dependence, even when taken as prescribed. Risk generally increases with higher doses and longer use. Abrupt stopping or a rapid dose reduction can cause life-threatening withdrawal, including seizures, so any reduction should follow an individualized gradual plan.

Alprazolam can cause dangerous sedation and breathing depression when combined with opioids, alcohol or other central nervous system depressants. Suspected overdose, very slow breathing, collapse or inability to wake requires emergency help.

Brain And Cognitive Effects

Memory, attention and decision-making can be impaired during treatment, and some people report persistent symptoms during prolonged withdrawal. Tolerance to some effects can develop, but taking more without medical direction increases harm. Physical dependence and withdrawal are not, by themselves, proof of addiction.

Liver And Kidney Conditions

Alprazolam is mainly metabolized in the liver. Liver impairment and medicines that strongly affect CYP3A can raise exposure. Therapeutic alprazolam is not established as a cause of progressive kidney damage, but kidney or liver disease should still be disclosed so the prescriber can assess dose, interactions and monitoring.

Short-Term And Longer-Term Effects

By enhancing GABA-A receptor activity, alprazolam can reduce anxiety while also causing drowsiness, slowed reactions, impaired coordination and reduced alertness. Longer use can produce physical dependence and can be associated with memory, concentration or emotional symptoms.

Memory And Cognitive Change

Anterograde memory impairment (difficulty forming new memories) is a recognized benzodiazepine effect, particularly with greater exposure or other sedatives. Persistent problems should be assessed. Current observational evidence does not prove that alprazolam causes dementia.

Mood, Rebound Symptoms And Unusual Reactions

Depression can worsen in some patients, and unusual agitation, hallucinations or suicidal thoughts need prompt medical attention. Rebound anxiety and panic can occur as the drug wears off or after rapid dose reduction; this may reflect withdrawal or return of the underlying condition rather than addiction alone.

Safer Use

  • Take alprazolam exactly as prescribed. Do not change the dose or timing on your own.
  • Avoid alcohol, and do not combine alprazolam with opioids or other sedatives unless the prescribers have explicitly coordinated the treatment.
  • Tell the prescriber and pharmacist about all medicines, supplements and substances because important CYP3A and sedative interactions can occur.
  • Do not drive or operate machinery while drowsy, dizzy or cognitively impaired.
  • Review ongoing benefit and adverse effects regularly. Water, food, exercise and “detox” products do not prevent dependence or make withdrawal safe.

Reducing Or Stopping Alprazolam

Do not stop suddenly after regular use. A taper should be individualized and adjusted to symptoms; there is no single safe schedule for everyone. The 2025 Joint Clinical Practice Guideline on Benzodiazepine Tapering emphasizes shared decisions, monitoring and treatment of the underlying condition.

Alternatives For Anxiety

Depending on the diagnosis and the individual, options may include cognitive behavioral therapy, an SSRI or SNRI, or other treatments. These are not guaranteed or interchangeable, and medicine changes require clinical guidance. Sleep, exercise and stress-management practices can support care but do not replace assessment or a safe taper.

Questions

Frequently Asked Questions

Can You Take Xanax With Liver Disease?
Liver impairment can slow alprazolam clearance and increase sedation or coordination problems. The prescriber may choose a lower dose, slower adjustment or another treatment. Tell the prescriber about liver disease and all medicines; do not change the dose yourself.
Can You Take Xanax On An Empty Stomach?
Alprazolam tablets are generally taken with or without food. Follow the product label and take each dose consistently as directed. Food is not a way to prevent dependence, interactions or overdose, and you should ask a pharmacist if nausea or other symptoms occur.
Does Xanax Make You Gain or Lose Weight?
Appetite and weight changes have been reported, but they are not predictable and may have many causes. Contact the prescriber about persistent or unexplained weight change rather than adjusting alprazolam on your own.
