What is Solpadeine? Solpadeine is a brand family and the ingredients and strengths can vary by product and country. Some formulations contain paracetamol with codeine, and some also contain caffeine. Indications depend on the exact product. UK codeine-containing Solpadeine products are for short-term relief of acute moderate pain not relieved by paracetamol or ibuprofen alone and are labeled for use for three days only. Read the pack and ask a pharmacist if pain persists. Examples of pain for which an appropriate formulation may be considered include:

  • Migraines
  • Earache
  • Tooth pain
  • Joint pain
  • Muscle pain
  • Back pain
  • Period pain
  • Sinus pain
  • Arthritic pain 

People tend to think that over-the-counter (OTC) medicines are completely safe. They’re available without a prescription, so they must be harmless.

Many people take these medicines without awareness of precaution toward the medication’s effects and side effects. This can lead to taking higher doses in an abusive manner and increasing the potential for harmful effects.

For codeine-containing Solpadeine products, the main safety issues come from codeine, paracetamol and any additional ingredients in the specific formulation. Codeine can cause drowsiness, constipation, nausea, respiratory depression, physical dependence and opioid-use disorder; paracetamol can cause serious liver injury if the recommended total dose is exceeded.

Common effects of codeine-containing products can include:

  • drowsiness or dizziness
  • nausea or vomiting
  • constipation
  • reduced alertness, especially with alcohol or other sedatives

More serious risks depend on the exact formulation. Important examples include:

  • slow or difficult breathing, extreme drowsiness or loss of consciousness from codeine toxicity
  • serious liver injury from excessive paracetamol, including accidental double-dosing with other paracetamol-containing products
  • physical dependence and withdrawal after regular codeine use

Paracetamol is present in many combination pain, cold and flu products, so accidental double-dosing can occur when people take more than one paracetamol-containing medicine. The main safety concern is exceeding the recommended total dose, which can cause serious liver injury. Someone who has taken more than the recommended amount should seek urgent poison or medical advice even if they initially feel well, because early symptoms can be mild or absent.

Codeine can cause tolerance and physical dependence during regular use, and some people develop opioid-use disorder. Needing a gradual reduction or experiencing withdrawal does not by itself prove addiction. Concerning signs include loss of control over use, taking more or more often than intended, persistent craving, obtaining extra supplies, or continuing despite clear harm.

Dependence and adverse effects need separate assessment:

After regular or long-term use of a codeine-containing co-codamol product, abrupt discontinuation can cause opioid-withdrawal symptoms. Onset and duration vary with dose, duration, formulation and individual factors rather than following a universal two-phase timetable. If codeine has been used for more than a short period, discuss a gradual reduction with the prescriber.

The symptoms of Co-codamol withdrawal include:

  • trouble sleeping
  • teary eyes
  • runny nose
  • sweating
  • yawning
  • loss of appetite
  • nausea and vomiting
  • stomach cramps
  • diarrhea
  • feeling irritable or anxious
  • trouble sleeping

Withdrawal severity and duration vary. Persistent or worsening symptoms need assessment, and experiencing withdrawal does not mean the person was abusing the medicine.

Caffeine can also lead to unwanted side effects. In high doses, caffeine can cause insomnia, nervousness and restlessness, stomach irritation, nausea and vomiting, increased heart rate and respiration, and other side effects.

The Side Effects of mixing Solpadeine and Alcohol

Caffeine can also produce tolerance and physical dependence in regular users. Stopping suddenly may cause headache, fatigue, irritability, low mood or difficulty concentrating. These symptoms are usually self-limited; people who consume substantial caffeine and want to avoid them can reduce intake gradually. Caffeine withdrawal should be kept distinct from opioid withdrawal caused by codeine.

Medications that are intended to heal can easily become dangerous when taken recklessly, especially those labeled as safe. Safe does not mean harmless and even with OTC medications, awareness and care are crucial when it comes to consumption.
