Sjukdomen anorexi (Anorexia nervosa) är en ätstörning som innebär att personen i fråga försöker svälta sig själv för att gå ner i vikt. Detta trots att personen redan väger mindre än vad som anses vara normalt och hälsosamt för dess ålder och längd. Trots att anorexi kan förekomma i alla åldrar så är det vanligast att få anorexi i tonåren. Personer som har anorexi har ofta en förvriden kroppsbild och anser sig själva vara tjocka trots att de är mycket smala på grund av det låga intaget av mat. Det finns flera tecken som tyder på att en person har anorexi och det kan finnas olika orsaker till att en person får anorexi. Sådan tur är finns det också behandling att få för denna sjukdom och ofta leder behandling till att man blir frisk från sjukdomen. Allt detta kommer vi att gå in på i lite mer detalj här nedanför.
Utveckling av anorexi kan ske antingen snabbt eller successivt över en längre tid. Ofta kan omgivningen misstänka att en person har anorexi eftersom personen blir väldigt smal på grund av det låga matintaget. Det finns flera besvär som en person kan uppleva som kan tyda på att personen har anorexi. Detta inkluderar till exempel att personen hela tiden tänker på dess matintag och vilken mat som bör och inte bör ätas.
En person med anorexi har ofta ett väldigt lågt matintag eftersom personen är mycket rädd för att gå upp i vikt och därför är det också vanligt att personen tränar mycket. Trots att många i omgivningen kan kommentera att personen är smal så anser personen sig själv vara tjock oavsett. Ofta är det svårt för personen i fråga att själv inse att man har anorexi, men om man känner igen sig i besvären ovan kan det tyda på att man har anorexi eller är i riskzonen för att utveckla anorexi.
Det finns ännu inte någon tydligt klarlagd orsak till att personer får anorexi. Som med de allra flesta psykiska sjukdomar är orsaken en kombination av ärftliga och miljömässiga faktorer. Det har till exempel visat sig att personer med en förälder eller ett syskon som har anorexi själva har en ökad risk för att få sjukdomen. Dessutom kan personer i ens omgivning med ätstörningar påverka ens egen syn på mat och ens kropp. I grund och botten handlar anorexi om att personen i fråga uppfattar sig själv som överviktig och inte är nöjd med sin kropp, varav personen känner ett ständigt behov av att gå ner i vikt. Detta kan i sin tur bero på att personen har ångest över att kroppen förändras under puberteten eller på grund av de orealistiska kroppsideal som finns i samhället. Det kan också bero på att personen har en låg självkänsla samt höga krav på sig själv att lyckas och prestera.
Anorexi har flertalet negativa konsekvenser på hälsan. Det låga matintaget gör att kroppen inte får den näring som den behöver för att hålla kroppen hälsosam och fungerande. Hur skadligt anorexi är för kroppen i den enskilda individens fall beror på hur snabbt personen har minskat i vikt samt hur länge personen har haft anorexi. Till exempel kan en väldigt snabb viktnedgång vara skadligare än en mer successiv viktnedgång eftersom kroppen då har mer tid att anpassa sig till förändringen. En person som får anorexi i tonåren då en stor del av kroppstillväxten sker kan bland annat få en minskad längdtillväxt och tjejer kan få en oregelbunden eller utebliven mens på grund av att hormonsystemet rubbas. Dessutom leder bristen på viktiga näringsämnen till att ämnesomsättningen sjunker. Detta kan också påverka längdtillväxten om personen befinner sig i ett tillväxtstadie och det kan också orsaka benskörhet samt vara skadligt för hjärtat som liksom andra organ kräver vissa ämnen för att fungera korrekt.
Anorexi har också en stor påverkan på magen och tarmarna. Det är bland annat vanligt att få problem med förstoppning, ont i magen och mycket gaser eftersom det låga matintaget leder till att mag- och tarmmusklerna försvagas varav tarmrörelserna minskar. Bristen på näring kan också leda till järnbrist och därmed blodbrist, vilket kan göra att personen i fråga blir väldigt trött.
Ett lågt matintag under en längre tid leder också till att kroppen ständigt försöker spara på den energi som finns, vilket bland annat sker genom att kroppen börjar stänga av viktiga kroppsfunktioner. Detta leder bland annat till att blodcirkulationen och pulsen minskar samt att kroppstemperaturen sjunker varav personen blir frusen. Dessutom kan personen få ett lägre blodtryck vilket i vissa fall kan leda till svimning och yrsel. Musklerna och skelettet blir också försvagat och dessutom kan håret och hyn påverkas och bli mer torrt. Vid en längre tid av svält kommer även hjärnan att påverkas och förlust av hjärnvävnad kan förekomma, vilket påverkar hur väl hjärnan fungerar.
Förutom dessa mer fysiska konsekvenser av anorexi, så är det också vanligt att flertalet psykologiska symtom förekommer samtidigt med anorexi. Det är bland annat vanligt att ha ångest, nedstämdhet eller depression vid anorexi. Detta kan i sin tur leda till att man isolerar sig själv och får en allt mer negativ självbild. Det är också vanligt att tvångstankar, fobier eller självskadebeteende förekommer tillsammans med anorexi och detta kan dessutom övergå i självmordstankar.
På grund av de negativa konsekvenser som anorexi kan ge upphov till är det viktigt att få behandling för sjukdomen. Det positiva är att de flesta som får behandling för anorexi blir friska och kan återgå till att leva ett hälsosamt liv både psykiskt och fysiskt. Var man får behandling samt vilken specifik typ av behandling man får beror lite på hur gammal man är. Personer under 18 år kan till exempel få hjälp på en ungdomsmottagning eller barn- och ungdomspsykiatrin (BUP). Personer över 18 år kan istället bland annat kontakta en psykiatrisk mottagning.
Oavsett ålder är det dock vanligt att behandlingen av anorexi börjar med att stoppa undernäringen och att patienten tillsammans med behandlaren kommer överens om ett matprogram att följa. Det är viktigt att detta är första steget så att patientens kropp och mentala hälsa hinner återhämta sig något innan behandlingen kompletteras med terapi. Terapin för ätstörningar som anorexi hos vuxna utgörs ofta av kognitiv beteendeterapi för ätstörningar (KBT-E). Terapin syftar till att patienten ska förstå hur dess tankar, känslor och beteenden hör ihop och ge patienten verktyg för att kunna bryta negativa tankemönster och beteenden och istället träna på att införa nya mer hälsosamma tankemönster som i sin tur kan leda till mer hälsosamma beteenden.
Barn och ungdomar upp till 18 år erbjuds ofta behandling mot anorexi som baseras på metoden Familjebaserad behandling (FBT). Denna behandling har visat sig vara den metod som ger bäst resultat för barn och ungdomar med ätstörningsproblematik. Denna behandling grundar sig i att hjälpa föräldrarna att ta ansvaret för att barnets matintag ska fungera och att skapa ett regelbundet matschema för barnet. Detta är viktigt så att barnet kan återfå en normal vikt och bryta ätstörningsmönstret.
Vanligtvis sker därför behandlingen i hemmet och det är rekommenderat att barnet till en början är hemma från skolan samt avstår från all fysisk träning för ett förbättrat behandlingsresultat. Om barnet behöver extra stöd med matintaget erbjuder flertalet mottagningar dagvård där barnet kan inta måltider tillsammans med föräldrar och behandlare. Vissa mottagningar kan ibland även erbjuda anhörigutbildning om anorexi för föräldrar och närstående för bättre förståelse om sjukdomen och hur den kan hanteras.
Vid allvarligare fall av anorexi kan det vara nödvändigt att patienten får behandling på ett behandlingshem som erbjuder vård dygnet runt. Då får patienten ofta bo i ett eget rum på behandlingshemmet med eller utan en förälder beroende på ålder. Behandlingen på ett sådant behandlingshem består ofta av terapi i form av kognitiv beteendeterapi för ätstörningar (KBT-E) och olika typer av måltidsstöd för att kunna återfå förmågan att inta mat på ett hälsosamt sätt.
