ARFID upptogs officiellt i DSM-5 år 2013, vilket innebär att den fortfarande är relativt ny i svensk sjukvård. Det betyder att många vårdgivare ännu inte är helt uppdaterade kring vad diagnosen innebär.
I Sverige är det framför allt BUP, barn- och ungdomspsykiatrin, som tenderar att uppmärksamma symptomen. Men, i takt med att diagnosen blivit mer känd för gemene man erbjuds det idag fler specialistmottagningar för vuxna.
En viktig aspekt är att ARFID inte påverkar alla på samma sätt. En del äter väldigt ensidigt men får i sig tillräckligt med näring, medan andra riskerar undernäring. Därför behövs en individuell bedömning för att ställa rätt diagnos.
När det kommer till symptomen vid ARFID kan dessa skilja sig kraftigt beroende på ålder. Barn visar ofta starka reaktioner på nya smaker eller konsistenser. Vuxna däremot har ofta anpassat sin vardag så att symptomen inte märks utåt.
De kan till exempel undvika alla sociala situationer där mat är inblandad – som restaurangbesök eller släktmiddagar. Många med ARFID har också psykosomatiska reaktioner, som illamående, huvudvärk eller magont vid blotta tanken på viss mat.
Det är heller inte ovanligt att personer som lider av ARFID genom sin uppväxt och vuxna ålder blivit beskyllda för att ha en kräsen inställning till mat, utan att någon utomstående förstått vidden av problemet.
ARFID är en ätstörning men inte kopplad till kroppsuppfattning eller önskan att gå ner i vikt. Detta innebär tyvärr att personer med ARFID ofta missas i vården, särskilt eftersom de inte uppfyller de klassiska tecknen på ätstörning.
Detta kan leda till att diagnosen ställs först efter flera år eller decennier av ohälsa och felaktiga behandlingar. Det finns dessutom överlapp med andra neuropsykiatriska tillstånd som ADHD och autism, där matintag också kan vara en utmaning.
Att skapa sig en helhetsförståelse gällande skillnaden mellan ARFID och till exempel sensoriska svårigheter kräver ofta en tvärprofessionell utredning där både medicinska, psykologiska och näringsrelaterade faktorer noggrant bedöms tillsammans.
För att kunna få diagnosen krävs en grundlig medicinsk och psykologisk bedömning. Det är vanligt att både barn och vuxna genomgår tester hos dietist, psykolog och ibland även logoped. Syftet är att förstå vilka faktorer som ligger bakom det undvikande beteendet.
Vid utredning granskas även tillväxtkurvor, blodvärden, ätbeteendehistorik och eventuella tidigare trauman. Hos vuxna kan det vara extra svårt att fastställa diagnosen, eftersom ätbeteendet ofta har blivit en “normalitet” för individen efter många år av undvikande.
ARFID diagnoskriterier enligt DSM-5
Kriterierna i DSM-5 fokuserar inte bara på maten utan också på konsekvenserna. Om en person till exempel måste ta näring via sond eller tillskott på grund av sitt undvikande matbeteende kan det räcka som underlag för diagnos.
Utöver detta ska symptomen inte bero på andra medicinska tillstånd eller kulturella vanor. Det är också viktigt att andra ätstörningar, som anorexia, utesluts innan diagnosen ARFID ställs. Diagnoskriterierna kan därför ibland kännas strikta – men de är nödvändiga för att undvika feldiagnoser.
Hos vuxna kan ARFID ofta förväxlas med selektiv ätproblematik eller social ångest. Det är inte ovanligt att vuxna patienter upplever skam inför sitt matbeteende. Det kan leda till att de undviker att söka hjälp – eller döljer sina symptom i vårdkontakter.
Många vuxna som får diagnosen berättar att de redan som barn hade ett mycket begränsat ätmönster, men att de aldrig blivit tagna på allvar. Med rätt stöd går det dock att bryta dessa mönster, även i vuxen ålder. I Sverige erbjuds idag flera behandlingsmetoder som är anpassade för vuxna.
Behandlingen fokuserar ofta på att minska rädslan kring mat, samtidigt som näringsintaget förbättras. Exponering sker ofta långsamt och kontrollerat och det är viktigt att personen själv är med och styr själva processen.
En vanlig metod är att införa “säker mat” som utgångspunkt och sedan stegvis bredda variationen. I vissa fall behövs kompletterande behandling för underliggande ångest, depression eller autism. Dietister spelar en nyckelroll för att säkra att kroppen får i sig det som krävs, medan psykologer jobbar med den mentala delen.
Lista: Vad du kan förvänta dig i en behandling

Behandlingstiden varierar men kan pågå under flera månader upp till år. Viktigast är att skapa hållbara förändringar, inte snabba lösningar. Sök hjälp via vårdcentralen eller specialistmottagningar som Stockholms centrum för ätstörningar.

Ju tidigare du får hjälp, desto bättre är förutsättningarna för att återgå till en balanserad livsstil.

ARFID kan leda till att du isolerar dig socialt. Du kanske tackar nej till fester, jobbresor eller till och med undviker relationer. Många beskriver att hela deras liv cirkulerar kring matundvikandet – och det tar mycket energi.
Även fysiska besvär som förstoppning, trötthet, frusenhet och koncentrationssvårigheter är vanligt. Vardagen blir begränsad, både psykiskt och fysiskt. Därför är det viktigt att ARFID tas på allvar som den sjukdom det är.
Att vara anhörig kan vara utmanande, men spelar en viktig roll inom sammanhanget. Det handlar inte om att tvinga personen att äta viss mat, utan att skapa trygghet. Tvingande beteende ökar ofta rädslan – vilket förvärrar symptomen.
Om du är förälder kan du samarbeta med skolpersonal och vård för att skapa en trygg struktur runt måltider. För vuxna med ARFID är det viktigt att ha någon som lyssnar och uppmuntrar – utan att pressa.
ARFID är fortfarande relativt okänt bland allmänheten, men svensk forskning visar att diagnosen är betydligt vanligare än tidigare trott.
