Den underliggande orsaken till alkoholabstinens är dock att en person som har haft ett långvarigt och intensivt alkoholintag abrupt slutar inta alkohol eller minskar på sitt alkoholintag. Alkoholabstinensen uppkommer då eftersom kroppen har påverkats på olika sätt av det ständiga alkoholintaget och blivit så van vid alkohol att kroppen inte fungerar normalt utan alkoholen. Det är på grund av att kroppen inte fungerar normalt som dessa abstinensbesvär uppkommer. För att bättre förstå varför dessa abstinensbesvär uppkommer bör vi först titta lite närmare på hur alkohol påverkar kroppen.
Alkohol påverkar hjärnan bland annat genom att hämma så kallade NMDA-receptorer och stimulera GABAA-receptorer. NMDA-receptorer förekommer naturligt i det centrala nervsystemet och binder signalsubstansen glutamat som har en aktiverande effekt på det centrala nervsystemet. GABAA-receptorer förekommer också i det centrala nervsystemet och dessa binder istället signalsubstansen GABA som har en hämmande effekt på det centrala nervsystemet. Eftersom alkohol hämmar NMDA-receptorer så kommer det förhindra inbindningen av glutamat och därmed minskar den aktiverande effekten på det centrala nervsystemet. Samtidigt stimulerar alkohol GABAA-receptorer vilket ökar inbindningen av GABA och detta bidrar till en hämmande effekt på det centrala nervsystemet. En minskning av glutamats aktiverande effekt och en ökning av GABA:s hämmande effekt innebär att alkohol totalt sett minskar aktiviteten i det centrala nervsystemet.
När en person har haft ett långvarigt intag av alkohol kommer alkohol att ha en stor påverkan på dessa receptorer i det centrala nervsystemet. Alkoholens hämmande effekt på NMDA-receptorer och stimulerande effekt på GABAA-receptorer kommer leda till en uppreglering av NMDA-receptorer och en nedreglering av GABAA-receptorer i kroppen för att kompensera för alkoholens effekt på dessa receptorer.
Så länge alkohol intas kontinuerligt kommer inte detta att kännas av. När en person å andra sidan avbryter ett långvarigt alkoholintag så kommer kroppens uppreglering av NMDA-receptorer att leda till att en större mängd glutamat binder in till dessa receptorer, vilket ökar aktiviteten i det centrala nervsystemet. Samtidigt kommer nedregleringen av GABAA-receptorer att leda till en minskad inbindning av GABA och eftersom GABA har en hämmande effekt på det centrala nervsystemet så kommer denna hämmande effekt att utebli. Därmed kommer avbrytandet av alkoholintaget att leda till att aktiviteten i det centrala nervsystemet ökar mycket och det är detta som tros vara orsaken till de symtom som uppkommer vid alkoholabstinens.
Alkohol verkar också genom att öka aktiviteten i hjärnans dopamin- och noradrenalinsystem. Därmed kommer nivåerna av dopamin och noradrenalin att öka i kroppen då alkohol intas. Dopamin är en viktig del i hjärnans belöningssystem och både dopamin och noradrenalin bidrar till välmående känslor. Ett långvarigt intag av alkohol kommer dock leda till att kroppens egen produktion av dopamin och noradrenalin minskar. När en person avslutar ett långvarigt alkoholintag kommer därmed nivåerna av dopamin och noradrenalin att vara väldigt låga i kroppen till en början och detta tros också ge upphov till alkoholabstinens. Både dopamin och noradrenalin har en stor påverkan på humöret och därmed kan en låg nivå av dessa signalsubstanser bland annat orsaka nedstämdhet och depression.
Vilka är då de abstinensbesvär som kan uppkomma vid alkoholabstinens? Trots att det finns vissa generella symtom som ofta uppkommer så kan dessa ändå variera från person till person. De abstinensbesvär som uppkommer är både fysiska och psykologiska och de beror på de fysiologiska förändringar i kroppen som det långvariga alkoholintaget har givit upphov till.
Vanligt förekommande fysiska abstinensbesvär inkluderar hjärtklappning, tremor, svettningar, takykardi, högt blodtryck, sömnsvårigheter, illamående, kräkningar, huvudvärk, aptitlöshet, utvidgade pupiller och trötthet.
Det är också vanligt att flertalet psykologiska symtom upplevs och humöret påverkas ofta i hög grad. Psykologiska symtom inkluderar nedstämdhet, depression, irritation, ångest, nervositet, agitation, hallucinationer, yrsel, humörsvängningar samt svårigheter att tänka klart.
Alkoholist personlighetsförändring
I vissa fall kan mer allvarliga abstinensbesvär uppkomma och ett av de mest allvarliga besvären kallas för ”delirium tremens”. Detta besvär uppkommer hos ungefär 3-5% av de som avslutar ett långvarigt intensivt alkoholintag och det uppträder ofta 2-4 dygn efter att alkoholintaget avslutats. De symtom som är karakteristiska för delirium tremens är hallucinationer, förvirring, feber, agitation, anfall och högt blodtryck. Detta tillstånd kan i vissa fall ha dödlig utgång och det är därför viktigt att det direkt behandlas i sjukvården.
Det finns flertalet olika behandlingar att tillgå vid alkoholabstinens för att lindra symtomen och förhindra att en person återgår till alkoholmissbruk. Vilken typ av behandling som ges beror på hur allvarliga abstinensbesvären är. Om symtomen är milda kan det till och med gå bra att stanna hemma utan behandling så länge man har någon som tar hand om en och ser till att symtomen inte blir värre. I vissa fall kan också bensodiazepiner skrivas ut eftersom dessa läkemedel har en sederande och lugnande effekt. Därmed kan lindra de värsta abstinenssymtomen samtidigt som de minskar risken för komplikationer som kramper och delirium tremens. Så länge bensodiazepiner används under en kortare period har de relativt få biverkningar och risken för beroendeutveckling är låg så länge läkemedlet trappas ut så snart den akuta alkoholabstinensen är över. Oftast används bensodiazepinen oxazepam i första hand men om det läkemedlet inte ger tillräcklig effekt används ofta diazepam.
Vilka är tecknen och symtomen på alkoholism?
Om abstinensbesvären är alltför svåra blir personen i fråga ofta inlagd på sjukhus där personen hålls under uppsikt och ibland ges också vätska intravenöst för att förhindra uttorkning. Dessutom kan läkemedel ges intravenöst för att hjälpa patienten att ta sig igenom de värsta abstinensbesvären eller för att behandla anfall eller andra allvarliga komplikationer.
Hur länge alkoholabstinens varar kan variera från person till person då det bland annat beror på hur högt alkoholintaget har varit och hur långvarigt alkoholintaget har varit. Generellt brukar abstinensbesvären ofta uppkomma inom åtta timmar efter sista alkoholintaget men vissa upplever inte symtomen förrän flera dagar senare. Abstinensbesvären brukar vara som värst 24-72 timmar och gå över inom några dygn efter det senaste intaget av alkohol, men mildare abstinensbesvär kan vara mycket längre än så hos vissa personer.
Som nämnt uppstår alkoholabstinens då ett långvarigt intag av alkohol avslutas eller minskas abrupt. Anledningen till att denna alkoholabstinens uppstår beror på att kroppen har blivit van vid alkoholen och kroppens fysiologi har förändrats på grund av alkoholen. Som tidigare nämnts hämmar alkohol NMDA-receptorer och stimulerar GABAA-receptorer. När en person har intagit alkohol under en längre tid kommer kroppen därför att uppreglera NMDA-receptorer och nedreglera GABAA-receptorer för att kompensera för alkoholens effekt på dessa receptorer. Denna fysiologiska förändring i kroppen leder till en ökad aktivitet i det centrala nervsystemet och det är detta som i sin tur anses ge upphov till abstinenssymtom.
