- Kokain är en drog som i de flesta länder är olaglig att inneha och använda och det är den näst vanligaste missbrukade illegala drogen efter cannabis i Europa, men den är inte riktigt lika vanlig i Sverige.

- Kokain verkar genom att stimulera det centrala nervsystemet och har därför starka effekter på det centrala nervsystemet.
Kokain är en drog som i de flesta länder är olaglig att inneha och använda och det är den näst vanligaste missbrukade illegala drogen efter cannabis i Europa, men den är inte riktigt lika vanlig i Sverige. Drogen utvinns ur kokabuskens blad och substansen klassas som en drog på grund av dess starkt beroendeframkallande och stimulerande effekter. Kokain verkar genom att stimulera det centrala nervsystemet och har därför starka effekter på det centrala nervsystemet. Kokain tillhör en av de narkotika som ofta missbrukas i samhället eftersom drogen ger ett rus hos den som använder drogen.
Redan vid intag av små doser av kokain erhålls ett rus som leder till ökad energi, ökad vakenhet samt ökad medvetenhet hos personen som har intagit drogen. Dock varar inte detta rus särskilt länge och när ruset avtar ersätts det ofta snabbt av nedstämdhet, abstinens och en längtan efter att ta mer av drogen. Därför är det lätt att bli beroende av kokain. I denna artikel kommer vi bland annat att förklara hur intag av kokain kan leda till kokainpsykos, vad en överdos av kokain kan leda till samt hur länge kokainet finns kvar i kroppen efter intag.
Kokain finns i både pulverform och i kristallstruktur och det är kristallstrukturen av kokain som kallas för crack eller crack-kokain. Crack tillverkas genom att kokain och bikarbonat kokas tillsammans varpå en fast kristallstruktur bildas. Denna kristallstruktur kyls sedan ner och brytas till mindre bitar som sedan säljs som crack. Namnet ”crack” kommer just från det knakande ljudet som uppstår när crack röks eftersom crack-kristallerna då spricker vid upphettning. Därmed skiljer sig kokain och crack åt i hur de ser ut men också i hur de intas samt de effekter de ger i kroppen.
Kokain är ett fint, vitt, kristalliskt pulver och det intas därför ofta genom sniffning.. Trots att det inte är någon skillnad mellan hur crack och kokain verkar i kroppen så kommer crack att upptas snabbare i kroppen och nå hjärnan snabbare eftersom det intas via rökning. Intag via rökning leder till snabbare effekter än till exempel intag via sniffning som är vanligare vid intag av kokain. Därför uppnås ett kokainrus snabbare vid intag av crack men ruset varar också under en kortare tid. Därför kan intag av crack leda till ökad risk för missbruk och beroende.
Kokainpsykos är ett tillstånd som kan uppkomma hos personer som har använt kokain under en längre tid. Denna typ av psykos leder till att personen som använder kokain kan uppleva symtom som är vanligt förekommande vid olika psykiska sjukdomar som bipolär sjukdom och schizofreni. Vanliga symtom vid kokainpsykos inkluderar paranoia, ångest, disorganiserade tankar, illusioner, våldsamhet, ilska, hallucinationer, delirium och självmordstankar.
Generellt finns det speciellt tre symtom som ofta uppkommer vid kokainpsykos:
Paranoia: Paranoia är det huvudsakliga symtomet av kokainpsykos och det är väldigt vanligt att detta symtom upplevs vid långvarigt användande av kokain. Innan denna paranoia framträder kan personen uppleva misstänksamhet eftersom detta ofta uppkommer innan paranoia. Paranoia kan göra att personen beter sig aggressivt eller våldsamt mot andra personer.
Hallucinationer: Det är också vanligt att hallucinationer uppkommer vid kokainpsykos till följd av en lång tids användande av kokain. Hallucinationer kan leda till att personen blir paranoid och förvirrad och det kan också förhindra att personen klarar av att hantera verkliga faror. Hallucinationer kan till exempel leda till att personen ignorerar vad som händer i miljön runt om eftersom att personen istället hallucinerar om någonting som inte finns.
Förvirring: Förvirring är ytterligare ett vanligt symtom vid kokainpsykos som leder till falska övertygelser hos personen. Detta kan till exempel innebära att personen inte vet vem den är eller tror att den är någon som den inte är samt att personen tror att den äger saker som den egentligen inte äger. Dessutom kan förvirringen leda till grandiositet, vilket innebär att personen får ett väldigt högt självförtroende samt tror att den har mer makt och kunskap än personen egentligen har. Personen kan också börja ifrågasätta identiteten hos sina familjemedlemmar, vänner eller andra personer.
Hur lång tid en kokainpsykos varav varierar från person till person, men den kan vara under några timmar och upp till så mycket som några dagar eller veckor. Symtomen av kokainpsykos kan bli mer allvarliga ju längre tid en person använder kokain. För att behandla en kokainpsykos är ofta det första steget detoxifiering på ett sjukhus eller på en rehabfacilitet. Det är då viktigt att personen slutar använda kokain så att psykosen kan behandlas på rätt sätt. När detoxifieringen är färdig får personen ofta fortsatt behandling med terapi och vissa läkemedel kan också förskrivas för psykosen.
Överdosering av kokain sker då en person har intagit en väldigt stor mängd kokain och detta kan ge allvarliga hälsokonsekvenser. En normal dos av kokain anses ligga runt 25-100 mg och en måttlig överdos uppträder ofta om mer än 500 mg kokain intas. En kraftig överdos innebär intag över 1000 mg och om dosen överstiger 3000 mg kan överdosen vara livshotande. Eftersom kokain har en stark påverkan på det centrala nervsystemet kan en överdos ha flertalet konsekvenser. Beroende på hur stor överdosen är kan flertalet effekter på hjärtat och hjärnan uppkomma och dessa kan i vissa fall vara livshotande. Det är därför väldigt viktigt att direkt uppsöka vård om du misstänker eller vet om att du har överdoserat kokain eller vet om någon annan som har gjort det.
Vilka är biverkningarna av överdosering av kokain?
De vanligaste symtomen som uppkommer vid överdosering av kokain är hjärtklappning, intensiv svettning och kramper. Det är vanligare att kokain orsakar dödsfall vid långvarigt missbruk än vid akuta förgiftningar. De vanligaste orsakerna till dödsfall vid långvarigt missbruk är på grund av påverkan på hjärtat och hjärnan.
Ofta kan livshotande överdosering förekomma om personen sväljer ner en stor mängd kokain för att undvika att bli ertappad av polisen. En sådan akut överdosering kan leda till att aktiviteten i det sympatiska nervsystemet blir väldigt hög vilket kan orsaka högt blodtryck, takykardi, kallsvettning, rigiditet samt värmeslag. Dessutom är det vanligt att personen upplever excitation, hög oro, rastlöshet samt psykotiska symtom. Personen kan också få utvidgade pupiller samt uppleva ofrivilliga rörelser och bröstsmärtor. Dessutom finns det risk att personen drabbas av hjärnödem samt lungödem med andningssvikt och även kardiovaskulär kollaps och koma kan förekomma.
Vid intag på över 500 mg kokain kan följande symtom uppkomma:
- Konfusion
- Psykomotorisk oro
- Aggressivitet
- Psykos
- Mani
- Kardiovaskulär kollaps
- Rabdomyolys
- Njursvikt
- Leversvikt
Efter att en person har intagit kokain kommer kokainet stanna i kroppen och kunna detekteras i kroppen under en viss tid. Hur lång tid det tar innan kokainet går ur kroppen beror på flertalet olika faktorer. Dessa faktorer inkluderar följande:
Hur mycket kokain som har intagits: Det tar längre tid för kroppen att utsöndra en högre mängd kokain, varav ett större intag innebär att kokainet kommer kunna detekteras i kroppen under en längre tid
Hur ofta kokain används: Ju oftare kokain används, desto längre kan det finnas kvar i kroppen och desto längre tid tar det innan det utsöndras
Hur kokainet har intagits: Kokain som intas genom snortning eller genom applicering på tandköttet kan stanna kvar i kroppen under en längre tid jämfört med om kokainet röks eller injiceras.
Om kokainet har intagits tillsammans med alkohol: Om en person har druckit alkohol samtidigt som kokain har intagits utsöndras kokainet saktare och stannar därför kvar i kroppen under längre tid
Vikt: En person som har en högre andel kroppsfett kommer ha kvar kokainet i kroppen under en längre tid eftersom kokainet ansamlas i kroppsfett
Metabolism: Olika personer har olika snabb metabolism och eftersom metabolismen påverkar utsöndringen av ämnen ur kroppen kommer den också påverka hur snabbt kokainet utsöndras
Generellt kommer kokainet finnas kvar i kroppen i upp till 2 dygn efter att den sista dosen av drogen intogs. Om en person har intagit en mycket hög dos av kokain kan drogen dock finnas kvar i kroppen under mycket längre tid, upp till några veckor.
För att mäta om en individ har kokain i kroppen används ofta ett urintest. Hur långt efter det sista intaget av kokain som kokain kan detekteras i urinen beror bland annat på hur hög dos som intogs samt personens metabolism. Vanligtvis kan kokain detekteras i urin i upp till 4 dagar efter att den senaste dosen intogs.
Blodprov används också för att undersöka om en person har kokain i kroppen. Kokain finns inte kvar lika länge i blod som i urin. Därför kan kokain ofta detekteras i blodet i upp till 2 dagar efter den senaste gången som kokainet intogs.
Efter att en person har använt kokain under ett tag har kroppen blivit så van vid den ökade mängden dopamin som drogen bidrar med att kroppen själv har minimerat sin produktion av dopamin. Därmed kommer en person som börjar ta en lägre dos av kokain eller slutar ta kokain helt och hållet att ha en väldigt låg mängd av dopamin i kroppen och detta leder till att personen upplever abstinenssymtom. Denna abstinens kan uppkomma trots att en del kokain fortfarande finns kvar i användarens blod. Abstinensen kommer främst leda till en stark längtan efter att ta mer av drogen. Till skillnad från abstinenssymtom som ofta uppkommer från alkohol och andra substanser, så orsakar kokainabstinens oftast inte fysiska symtom som kräkningar och skakningar utan istället mestadels psykologiska symtom. De symtom som ofta uppkommer vid kokainabstinens inkluderar följande:
- Trötthet
- Brist på njutning
- Ångest
- Irritabilitet
- Sömnighet
- Rastlöshet
- En extrem känsla av misstänksamhet eller paranoia
- Koncentrationssvårigheter
- Depression
- Kallsvettningar, muskelsmärta och nervsmärta
