Obsessive compulsive disorder (OCD) är ett engelskt begrepp som beskriver det som vi på svenska kallar tvångssyndrom. Det är en rad beteende och tankar som tvångsmässigt utförs av den drabbade personen. Tvångssyndromet grundar sig i en ångestproblematik och är ett beteende som används för att kontrollera denna problematik.
Det finns behandling i form av terapi och även läkemedelsbehandling. Denna behandling är framgångsrik i de allra flesta fall. Detta innebär att man kan få bort de tankar och beteende som tvångssyndromet tar sig uttryck i eller minska dess effekter. Målet är alltid att hamna på en nivå där man kan ha en god livskvalitet, trots sin diagnos.
OCD är olika tvångsmässiga beteenden som kan grupperas under beteckningen tvåjngssyndrom. Det finns en närliggande personlighetsstörning med liknande symtom. Men personer som lider av denna personlighetsstörning ser inte sina tvångsmässiga beteenden och tankar som något negativt.
För personer med tvångssyndrom så är beteendet en reaktion på en underliggande ångest och därmed inte ett positivt beteende. Det är vanligt att beteendet orsakar ytterligare ångest, i kontrast till varför beteendet utförs alls. Tvångssyndrom kan både vara beteende och tankar och dessa kan snarare ses som ritualer.
Man delar in dessa i fyra huvudsakliga kategorier. Det finns beteendemönster och tankemönster som inte passar in i dessa fyra kategorier. Men huvuddelen av de personer som lider av syndromet kan placeras under en av dessa rubriker.
- Att kontrollera. Det kan handla om att man känner ett starkt behov av att kontrollera att ett lås är låst, att ett larm är igång eller av eller att upprepade gånger kontrollera om en ljusknapp fungerar. Det kan samtidigt handla om att kontrollera om man har kroppsliga problem. Som att ta blodprov för att se om man har en specifik sjukdom, att begära utredningar kring sjukdomar som man troligtvis inte har. Det kan också manifestera sig i att kontrollera om man är gravid ofta eller andra liknande beteenden.
- Smitta. Här handlar det om en rädsla att få en smitta och beteendet kan ta sig uttryck genom ständigt tvättande av händer eller att man städar ovanligt ofta. Det kan också märkas i hur en person diskar ren disk eller oroar sig kring dessa områden på ett ohälsosamt sätt.
- Symmetri och ordning. Personer som kan placeras i denna kategorin känner ett stark behov av att ha ordning om kring sig och att saker är symmetriska. Det kan vara att saker ska vara placerade på ett visst sätt eller att, exempelvis, gardiner är helt symetriska.
- Ältande och påträngande tankar. Denna kategori handlar om att man har tankar som tränger sig in i ens medvetande och som man ägnar mycket tid till att älta.
Som sagt så är dessa kategorier inte kompletta. Det händer också ofta att en persons tvångbeteenden kan hamna i dubbla kategorier.
Alla beteende och tankemönster kan bli tvångsmässiga. Det är egentligen inte i beteendet problemet ligger utan i den respons som beteendet utgör. I grunden så finns det en stark ångest och beteendet är en respons på denna ångest.
Som ett exempel kan vi ta de personer som oroar sig för att bli smittade. Där är ångesten kring smittan och vad det skulle leda till. Ångesten är i sin natur överdriven och tar sig inte uttryck i en normal nivå av undvikande av möjligtvis smittade ytor och situationer. Istället så tar det sig uttryck i beteende som inte är i nivå med risken.
Alltså exempelvis i överdrivet tvättande av händer och onormalt mycket städande av redan rena ytor. Detta beteende blir väldigt handikappande för personen. Det påverkar stora delar av personens liv och det sänker deras upplevda livskvalitet.
När det gäller OCD så är symtomen dels de beteende som det tar sig uttryck i. Vi hade ett exempel kring renlighet och smitta, men som vi såg i de olika kategorierna så kan det ta sig många uttryck. Samtidigt så är många av de symtom man upplever de tankar man har kring exempelvis smitta eller de tankar man har kring kontroll.
Det handlar även om en ständig vaksamhet kring det ens tvångsmässiga tankar och beteende kretsar kring. En person som har tvång kring renlighet kommer också vara överdrivet vaksam kring detta. Det kan handla om att man inte kan röra vid ledstängerna på bussen eller att det blir svårt att äta mat som man själv inte har tillrett.
För att hitta orsaken till tvångssyndromet och hur det tar sig uttryck måste man titta på den ångest som orsakat tvångssyndromet. Det kan vara en ångest som har sitt ursprung i psykisk ohälsa och uppstått som ett symtom på annan sjukdom. Men samtidigt kan den relatera till något mer specifikt i livet.
De som tvångsmässigt kontrollerar sin partner och är väldigt svartsjuka kan ha en ångest som handlar om en rädsla att bli övergiven. Den tar sig uttryck i detta kontrollbehov, ett behov som inte bara drabbar personen, men även dennas partner. Obehandlat så kan tvångssyndrom bli till stora handikapp som begränsar personen.
Samtidigt så kan man utveckla tvångssyndrom i vuxen ålder. Det kan vara en respons på en livskris som att man blir lämnad av sin partner eller att en nära anhörig går bort.
Det finns vissa riskfaktorer som ökar sannolikheten för att man ska utveckla ett tvångssyndrom. Läkarna är inte ense om varför vissa utvecklar tvångssyndrom och andra inte. Dock har man kunnat peka på att det finns vissa riskfaktorer som ökar sannolikheten för att det ska utvecklas.
Stress är en av dessa riskfaktorer. Att utsättas för stor emotionell stress eller stress i vardagen kan skapa en situation där en person utvecklar dessa beteenden. Stress är även känt för att förvärra symtomen hos de personer som redan har diagnosen. Man har även sett att OCD är något mer vanligt förekommande hos kvinnor än hos män.
De första symtomen visar sig ofta redan i tonåren, men kan även dyka upp hos små barn. Det är viktigt att identifiera dessa tidigt så att man kan sätta in rätt behandling innan deras liv påverkas allt för mycket. Om en förälder har OCD eller ett syskon har det så är sannolikheten större för att man ska utveckla det själv.
Man har även kunnat identifiera fysiska skillnader i hjärnstrukturen hos de som utvecklar ett tvångssyndrom när man jämför med andra som inte har gjort det. Varför dessa skillnader uppstår vet man inte än. OCD kan även vara något som dyker upp efter trauman. Vi pratade tidigare om livskriser som riskfaktor. Forskarna menar att även trauman som barn kan utlösa problematiken.
Det kan handla om att man blivit utsatt för sexuella eller fysiska övergrepp i unga år. Man har även sett hur tvångssyndrom dykt upp hos barn efter att de fått en autoimmun sjukdom efter att ha drabbat av en streptokockinfektion.
Det är viktigt att man behandlar det som är orsaken till tvångssyndromet, men även att man behandlar själva beteendet. Med hjälp av kognitiv beteendeterapi så kan man arbeta kring responsen som beteendet är. Detta är ett bra utgångsläge eftersom beteendet ofta orsakar än mer ångest.
Det kan handla om att terapeuten som håller i KBT med klienten försöker ändra beteendet till något mindre destruktivt eller utåtagerande. Det kan även handla om så kallad responsprevention. Där försöker man hindra att beteende tar sig uttryck över huvud taget. Där efter försöker man arbeta med roten till ångesten. Detta kan vara svårt då orsaken kan ligga flera år tillbaka i tiden eller inte vara helt tydligt för personen.
Det går även att behandla personen med läkemedel. Då handlar det ofta om ångestdämpande läkemedel som SSRI eller SNRI. Dessa har bevisad effekt som ångestdämpande och har även visat sig ha god effekt på själva tvångbeteendet.
I de mest extrema fallen av tvångssyndrom kan man med hjälp av kirurgi på hjärnan komma åt problemet. Detta förutsätter at man kan se precis vart man ska utföra kirurgin. Det innebär också att man är så pass illa där an i sin sjukdom att fördelarna överväger nackdelarna. Med all kirurgi så finns det risker. När man opererar på hjärnan så är dessa stora. Om man kan ta de risker det medför så kan kirurgi vara ett alternativ.
Det är dock inte särskilt utbrett med kirurgi som behandling av tvångssyndrom idag. Sedan ett par år tillbaka avråder Socialstyrelsen helt ifrån kirurgi som behandlingsmetod. Dock finns det individer som åker utomlands för att få denna slags kirurgi utförd. I de fallen så står man själv för alla kostnader och man täcks inte av de statliga sjukförsäkringar vi har i Sverige.
I det dagliga livet så finns det en rad olika avslappningsövningar att göra för att kontrollera och minska problemen. Det kan handla om mindfulnessövningar eller ren meditation. Många personer med tvångssyndrom uppger att detta är ett bra komplement till den mer traditionella terapin och att det påverkar deras liv till det bättre.
Hur det är att leva med OCD varierar från person till person. Det är alltså mycket individuellt. Vissa har som lider av tvångssyndrom lever goda liv där det inte påverkar livskvaliteten i någon större utsträckning. Samtidigt så finns det personer som inte kan leva normala liv på grund av detta tillstånd.
Deras rädsla för att bli smittade, deras kontrollbehov, ordningsbehov eller ältande tar så pass mycket plats att det påverkar de flesta delarna i deras liv. Ofta har dessa personer något slags stöd från samhället. Det kan handla om att man fått plats på någon form av boende, exempelvis i enlighet med LSS-lagstiftningen. Det kan också innebära att man får hjälp i hemmet för att klara de vardagliga bestyren.
Om en person som har medelsvår eller svår OCD får rätt terapi och medicinering så är förhoppningarna att de ska kunna leva goda liv. OCD är utbrett i befolkningen men, som vi konstaterat, så är graden av svårigheter man tvingas leva med varierande.
