- Social fobi är en diagnos som innebär att en person får svår ångest och rädsla i sociala sammanhang och då personen får uppmärksamhet från andra personer.
- Detta kan till exempel handla om att träffa nya personer, att prata inför en grupp människor eller att prata i telefon med människor.
- Det är vanligt att en person som har social fobi upplever följande symtom i större grupper av människor: hjärtklappning, rodnad, svettningar, muntorrhet, klump i magen, darrningar och yrsel.
Social fobi är en diagnos som innebär att en person får svår ångest och rädsla i sociala sammanhang och då personen får uppmärksamhet från andra personer. Det är vanligt att en person som har social fobi upplever följande symtom i större grupper av människor: hjärtklappning, rodnad, svettningar, muntorrhet, klump i magen, darrningar och yrsel. Ofta vill man inte att andra personer ska märka av dessa symtom, varav personer med social fobi ofta isolerar sig från andra.
Social fobi handlar alltså inte bara om att vara blyg eller ha prestationsångest, utan det innebär ett större obehag för personen som lider av det och en stor påverkan på livet. Detta leder ofta till att personer med social fobi undviker sociala sammanhang och människor i stort, vilket ofta påverkar ens vardag väldigt mycket.
När det gäller social fobi så anses både arv och miljö bidra till utvecklingen av diagnosen. Det är inte helt klarlagt än om det finns vissa faktorer som bidrar mer till utvecklingen av social fobi än andra, men nedanstående faktorer ökar risken för att utveckla social fobi:
Ålder: Det är vanligt att social fobi först uppkommer i tonåren och de allra flesta utvecklar social fobi innan de är 20 år
Kön: Kvinnor utvecklar social fobi i högre grad än män
Andra sjukdomar: Personer som har social fobi och söker hjälp för det har oftast också depression och andra ångestsyndrom. Det är speciellt vanligt att personer som lider av social fobi även har generaliserat ångestsyndrom.
Personlighet: Om man var blyg och dämpad som barn så har man en ökad risk för att utveckla social fobi under tonåren
Hemmiljö: Det har visat sig att föräldrar som är överbeskyddande och starkt kritiska har förknippats med utvecklande av social fobi hos barn, men det är ännu inte en klar orsakande faktor
Tidiga negativa upplevelser: Det är vanligt att utsatthet för jobbiga sociala händelser under barndomen, som att ha blivit utskrattad eller förnedrad inför en grupp, kan ha betydelse i utvecklingen av social fobi.
Fysiska faktorer: Det har visat sig att hjärnans aktivitet ändras vid social fobi, men det är ännu oklart vilken betydelse detta har.
Rädsla för att bli utstött: Social fobi kan också uppkomma på grund av en rädsla att bli utstött ur en grupp eller bli negativt dömd av andra. Denna rädsla har egentligen en evolutionär förklaring eftersom det evolutionärt sätt var väldigt viktigt för vår överlevnad att vara socialt accepterade och tillhöra en grupp.
Det kan vara väldigt begränsande att leva med social fobi, men det finns hjälp att få för att bli av med social fobi och därmed kunna leva ett mer fulländat liv. Det är idag vanligast att social fobi behandlas med kognitiv beteendeterapi (KBT), ibland i kombination med läkemedel.
KBT för social fobi
Kognitiv beteendeterapi används idag för flertalet olika psykiska sjukdomar och diagnoser och det anses vara den mest effektiva behandlingen vid social fobi. I KBT ligger fokus på att individen med social fobi ska kunna bryta den onda cirkeln av negativa tankar som är grunden för det beteende som leder till att personen undviker sociala situationer. Detta uppnås genom att personen under terapin får hjälp att klara av och stå ut med obehagliga tankar och känslor som kan uppkomma i sociala situationer.
Detta sker stegvis i en metod som kallas för exponering, vilket innebär att man succesivt får öka sin exponering för sådant som man annars brukar undvika och som framkallar ångest eller rädsla. En annan viktig del av behandlingen är att personen som lider av social fobi får lära sig om de fysiska symtom som ofta uppstår vid social fobi, varför dessa symtom uppstår samt hur personen kan lära sig att hantera dessa symtom och den ångest som uppkommer.
Läkemedel mot social fobi
Ibland kan psykologisk behandling som KBT behöva kombineras med läkemedel vid social fobi. Det är vanligast att social fobi behandlas med läkemedel som även används mot depression, det vill säga antidepressiva läkemedel som till exempel SSRI-preparat, som påverkar nivåerna av substansen serotonin i hjärnan.
Vissa personer som lider av social fobi har endast besvär vid vissa specifika situationer, som till exempel när de ska träffa nya människor eller prata inför en grupp människor. I dessa fall kan det räcka att ta läkemedel i dessa specifika situationer och dessa läkemedel är då ofta till för att minska symtom som hjärtklappning, svettningar, darrningar och yrsel. Då kan man till exempel få använda läkemedel som kallas för betablockerare som fungerar genom att lugna ner hjärtat och de hjälper därmed till att förebygga hjärtklappning, darrningar och yrsel.
Bensodiazepiner är en annan grupp av läkemedel som ibland övervägs vid behandling av social fobi. Detta är lugnande läkemedel som hjälper vid svår oro och ångest som ofta uppkommer i samband med social fobi. Dock har dessa läkemedel ofta också en sövande effekt och de kan också vara beroendeframkallande, varav de används med försiktghet och endast om annan behandling inte hjälper.
Det finns flertalet behandlingshem som erbjuder hjälp och behandling för personer som lider av social fobi. På behandlingshem består behandlingen oftast främst av kognitiv beteendeterapi (KBT) och man får ofta behandling både enskilt samt i grupp med andra personer som har liknande problem som en själv. Vissa behandlingshem kan erbjuda boende på behandlingshemmet, medan du på andra behandlingshem kommer in några dagar i veckan för behandling.
Förutom behandling i form av terapi eller läkemedel så finns det även flera saker som du själv kan tänka på för att aktivt arbeta med din sociala fobi och bli av med den. Det är till exempel hjälpsamt om du själv lär dig mer om hur ångest fungerar, varför det uppkommer och hur det påverkar dig och ditt beteende. Genom att veta mer om orsakerna till att du känner ångest och rädsla kan du vara mer kapabel till att ta kontroll över situationen och veta hur du kan hantera ångesten.
Försök även att tänka över om det är några specifika situationer du undviker och varför du specifikt undviker dem. Försök sedan att utmana dig själv till att stanna i en sådan situation trots att du vanligtvis undviker den. Reflektera sedan över om det verkligen var så farligt att stanna i situationen som du först trodde. Hände verkligen det som du trodde skulle hända eller var situationen kanske inte så farlig ändå?
Det kan dessutom alltid vara till stor hjälp att prata med någon som du har förtroende för, kanske en förälder eller en vän, och berätta hur du känner för att få hjälp och stöd. Ofta kan andra personer kanske ha upplevt liknande känslor själva och därför ha bra råd till dig om hur de överkom liknande problem. För övrigt kan ångest dämpas av regelbunden fysisk aktivitet och sömn eftersom detta stärker kroppen både psykiskt och fysiskt på flertalet sätt.
